I understand that a minimum notice period of 10 working days is required and that I am responsible for cancelling my direct debit instruction with my bank. I acknowledge that any family members linked to this membership will no longer be valid. I understand that this request does not constitute final cancellation and will be reviewed by a membership advisor.

Please note: It is important to give a minimum of 10 working days notice before your next due payment, to insure all charges have been suspended before collection date.
